Designation: Intake & Assessment Support Worker

Reporting to: Head Office and Logistics Coordinator 

The Overall objectives of the position are as follows:

The Intake and Assessment Support Worker will be responsible to execute all processes in relation to the assessment of prospective clients as well as be participative in discussions in relation to the acceptance of new clients into the Foundations’ residential homes. The Intake & Assessment Support Worker will follow the direction of the Head Office and Logistics Coordinator to ensure the best possible process in relation to Intake and Assessment. 

The Intake & Assessment Support Worker’s responsibility and accountability must constantly remain within the Foundation’s policies and procedures.

Specific Tasks

  • To be proactive in building an Intake & Assessment process that will benefit the Foundation.
  • Contribute ideas, suggestions, and observations regarding the effectiveness of the Intake & Assessment processes. 
  • Contribute ideas, suggestions and observations regarding the assessment tools used by the Foundation. 
  • To manage the Intake & Assessment e-mail address by liaising affectively and in a timely manner with referring professionals.  
  • To carry out assessments as effectively as possible and with the most efficiency, to gather enough knowledge for the best possible outcomes. If the need arises, the General Manager is also to be included in these discussions. 
  • To internally liaise with residential teams including the Residential Coordinator, Residential Social Worker and Residential Support worker prior to a decision being taken for new admissions. 
  • To keep track of all referrals in an efficient and transparent manner.
  • To liaise with Residential Coordinators to provide all necessary data to complete official reports in relation to Intake & Assessment. 
  • To attend all Case Load Meetings to remain familiar with the dynamics present in each residential home. 
  • To intervene with clients on a need basis.
  • To take initiative and be pro-active. 
  • To support other teams at the different residential Homes, as necessary.
  • To be involved and participate in other duties of the psychosocial team.
  • Follow training and development programmes, recommended by the Management. 
  • Be economical in usage of the Foundation’s budget and operate responsibly. 

Tasks in relation to the Client Management System (CMS) 

  • To act as administrator of the Client Management (CMS) system in place at the Foundation (CORTEX). 
  • To create client profiles for prospective clients upon receiving referrals. 
  • To liaise with Management in relation to the need of new reports to be created in the CMS according to the needs of the Foundation. 
  • To input data that is gathered from assessments and any other relevant documentation. 
  • To make sure that all accepted clients’ profiles are updated with the client’s photo.
  • From time to time to cross check that all data in relation to clients’ scheduled interventions are being inputted in a timely manner. 
  • To assist in the inputting of any needed and relevant data into the CMS. 

Personal Development

  • Participate in all development and training programmes as recommended by the Management.
  • Identify opportunities for personal and professional development and bring to the attention of the Head Office and Logistics Coordinator. 
  • The Intake & Assessment Support Worker should also strive to have good role performance and good communication with other staff members. 

Other Responsibilities.

The Intake & Assessment Support Worker may from time to time be called upon by the Management or designate to perform certain non-recurring tasks which, although not specifically spelled out within the text of the job description, are nonetheless akin to the primary goals and objectives of the job description of the Intake & Assessment Support Worker and are intended to enhance the end results and/or to overcome unexpected obstacles.

The Head Office and Logistics Coordinator will be available to assist the support worker in any need that may arise and, in any situation.
